Requesting Transportation Services by Phone

To schedule a ride once you have filled out the application

• Call the office at (574) 295-1820 between the hours of 8:00 a.m. and 4:30 p.m. Monday through Friday (except holidays) to schedule your ride.

  • Ride requests are required to be made at least 48 business hours in advance.

    • Call to schedule your ride as soon as you are aware of your needs

    • Rides can be scheduled up to 4 weeks in advance

    • Medical trips have priority in scheduling
      

• Have the following information ready when you call to schedule:

  • Your Name o Your Pick-up Address

  • Your Telephone Number o Medicaid Number (if applicable)

  • Time of your appointment

  • Name & Address of Destination

  • Pick-up time for your return trip

  • Whether you use a wheelchair, walker or any mobility device or have other special needs o Wheelchair width

  • If someone will be riding with you

  • If you have a service animal

  • If you will have a caregiver

Rides are available Monday-Friday, 6 a.m.– 6p.m. Special arrangements can be made to schedule trips outside of operating hours.

Suggested Donations for In Elkhart County Transportation is $5.00.

All Medicaid clients are required by the state to pay any required Co-Payment.
